You will need to show a certified copy as well as a photocopy of an official birth certificate (or some other acceptable document demonstrating proof of age). See below for acceptable documents.

Proof of Age

If you are registering for the soccer season, then you must be aware of the birth certificate (or proof of age) situation. Here is the latest on that.

ALL new players must provide proof of age before registration can occur. This means that you have to bring a certified copy of a birth certificate as well as a photocopy. We will keep the photocopy. There are other documents which can be accepted and that will be defined at the bottom of this note.

Also, note that for competitive players, the same holds true. You will have to provide these documents for the season if you have not played competitive soccer.

Documents needed for Proof of age:

f. Any player may be required to prove his legal age or legal residence to GCSA.

(1.) Proof of age shall consist of a birth certificate or birth registration issued by the appropriate government agency, passport, alien registration card issued by the Immigration and Naturalization Service attesting to age, or a certification of an American citizen born abroad issued by the appropriate government agency. Hospital, baptismal, shot records, or religious certificates shall not be accepted.

(2.) Legal residence is the true domicile of the player and his or her family/legal guardian. This is, the place where the player and his or her family/legal guardian have their permanent residence to which, if absent, they have the intention of returning.

Birth certificate for a child born in Oklahoma may also be obtained for $10 from the Tulsa Health Department on the corner of 3rd and Utica
(918-582-9355).  You will need a current photo id (driver's license is acceptable) to request the birth certificate.
